Are you showing any signs of Covid-19?

Including any of the following: a high temperature, a new continuous cough,
loss or change to sense of smell or taste.

If you are showing signs of new marks, rashes, spots, bumps or other lesions on your skin or toes, you should not receive treatment until you have sought medical advice.

Have you: been in contact, in the last 14 days, with someone who has been diagnosed with or suspected of having Covid-19 or been contacted by the NHS Test & Trace team and advised to self isolate?

If you have answered yes, you should not seek treatment at this time.

Are you likely to have been exposed to Covid-19 in any other way?

Considerations include: local lockdown restrictions, travel by public transport, recent travel to quarantined countries, or any other situation which might increase, or have increased, the risk of you becoming infected.

Were you defined by the Government as being "extremely vulnerable" and shielding, or "vulnerable"?
